Service Agreement Overview
Steward Care Solutions LLC provides non-medical in-home support services designed to assist individuals with daily activities and promote independence.
Scope of Services
Services may include, but are not limited to:
Personal care assistance
Companion care
Mobility support
Transportation assistance
Household support related to client care
Specific services are determined during consultation and documented in a care plan agreed upon by the client and Steward Care Solutions LLC.
Scheduling and Availability
Services are scheduled based on availability and client needs. Appointment times, caregiver assignments, and service frequency will be discussed prior to the start of services.
Client Responsibilities
Clients or responsible parties agree to provide accurate information regarding care needs and maintain a safe environment for caregivers.
Payment and Fees
Service fees, billing schedules, and payment methods will be discussed and agreed upon before services begin.
Modification or Termination of Services
Either party may request changes to services or discontinue services with appropriate notice as outline in the service agreement signed before care begins.
Independent Relationship
Caregivers assigned through Steward Care Solutions LLC provide supportive assistance but do not replace licensed medical professionals.
